    Equity, Diversity and Inclusion - Organization Assessment Tool (EDI-OAT)

    This tool will be used by employees of CLNH to help identify areas where the agency is doing well and the areas that need improvement in regard to equity, diversity and inclusion practices. Results obtained using the Assessment Tool will demonstrate the agency with the necessary information and allocation of equity, diversity and inclusion resources. Your identity will be kept anonymous and all answers will only be reviewed by an external third party, not Community Living North Halton. After completing the survey, you will have the opportunity to provide your email if you wish to receive a gift card in exchange for your participation. Your identity will still remain anonymous to Community Living North Halton, as a third party will distribute the gift card. This survey must be completed in one full session, and it will take you approximately 15 minutes to complete it. You will only have one opportunity to complete the survey.
    Definitions:

    Diversity
    is about the individual and about the variety of unique dimensions, qualities and characteristics, we all possess. Each of us was born into a society with a specific culture, which is the foundation of our beliefs, values, knowledge, genders, rituals and more.

    Equity is the quality of treating individuals fairly and bias freebased on their needs and requirements.

    Inclusion is about creating a culture that strives for equity and embraces, respects, accepts and values difference.

    Diversity and Inclusion is about capturing the uniqueness of the individual; creating an environment that values and respects individuals for their talents, skills and abilities to the benefit of the collective.
    Scoring Scale:

    During this self-assessment survey, you will be asked to answer with the following options: (1) Strongly Disagree, (2) Disagree, (3) Neutral, (4) Agree, (5) Strongly Agree, (0) Unsure and (0) Not Applicable (N/A).

    (1) Strongly Disagree means you strongly disagree with the statement.

    (2) Disagree means you disagree with the statement.

    (3) Neutral means you neither agree nor disagree with the statement.

    (4) Agree means you believe the statement to be true.

    (5) Strongly Agree means you strongly agree with the statement.

    (0) Unsure means you do not understand the question.

    (0) Not Applicable means you are certain that the question does not apply to you.
