Equity, Diversity and Inclusion - Organization Assessment Tool (EDI-OAT) Step 1 of 812%Community Living North Halton This tool will be used by employees of CLNH to help identify areas where the agency is doing well and the areas that need improvement in regard to equity, diversity and inclusion practices. Results obtained using the Assessment Tool will demonstrate the agency with the necessary information and allocation of equity, diversity and inclusion resources. Your identity will be kept anonymous and all answers will only be reviewed by an external third party, not Community Living North Halton. After completing the survey, you will have the opportunity to provide your email if you wish to receive a gift card in exchange for your participation. Your identity will still remain anonymous to Community Living North Halton, as a third party will distribute the gift card. This survey must be completed in one full session, and it will take you approximately 15 minutes to complete it.
